PennDOT Coroner’s Report of Motor Vehicle Death
In September of 2021, the Fatality Analysis Reporting System (FARS, formerly CARS) updated the AA-34 form, also known as the “Coroner’s Report of Motor Vehicle Death.”
This form must be submitted on all fatalities occurring in or as the result of a passenger vehicle.
The form must be submitted within five days of the certification of the death (whether the cause and manner are pending or not.)
Complete the form provided by the FARS Unit and return by email, fax, or mail.

THE VEHICLE CODE, SECTION 3749
§3749. Reports by coroners and medical examiners.
(a) General rule.—Every coroner or medical examiner in this Commonwealth shall report in writing to the Department within five days of certification the death of any person resulting from a vehicle accident, giving the time and place of accident and the circum- stances relating thereto. These reports shall be made on forms prepared by the Department. Every coroner or medical examiner shall retain a copy of the reports in his office for a period of two years.
